Delta Waves Sleep Disorders and Research Center in located just off of North Academy Boulevard in Colorado Springs, Colorado. Our office is open 8 am – 5 pm Monday – Thursday and 8 am – 3 pm Friday.
If you, or a loved one, suffers from the effects of a sleep disorder and would like to be seen by our Sleep Specialist or take part in a sleep study, appropriate documentation is necessary.
For a sleep study it is necessary for your physician to complete a sleep services order form (please see link below). Completed forms may be faxed to our scheduling department at 719-262-9285, along with your current insurance information & most recent clinic notes.
We do accept self –referrals for office consultations with our Sleep Specialist.
Once we have received the appropriate documentation, a member of our scheduling department will give you a call in order to schedule your appointment.
